Dear Parents,
Please find below the combined Data Collection and Medical Consent Authorisation Form 2025-26 for completion for your child. To ensure that the data that we hold for your child is up to date and to enable them to participate in 'off-site' activities in the new Academic Year, it is important that the form is completed as soon as possible.

Residency and Right to Study in the UK:
To study in the UK a child must have either British or Irish Nationality, Settled Status or a valid and in date immigration status that permits study. Please note that the Foundation does not have a sponsor licence.
If your child does NOT have a UK or Irish passport please provide details of your child's right to study in the UK
Consents and Declarations:
This form provides core information that the school will use to provide an appropriate level of medical care for your child through the school day and whilst away from the school on trips, visits or off-site activities (including QED for the Senior Section). It is therefore critical that, following completion of this form, during the academic year, if there are any changes to their health or contact information that you supply here, you must advise the school office/school nurse immediately so that the central record can be updated. The information collected here will be treated confidentially, adhering to the Data Protection Act, and only used for the purposes outlined above.
Emergency Contacts
Please provide the full name, salutation and contact number of the pupils emergency contacts below. Including parents/carers, we must have a minimum of 3 contacts on file for every pupil.
A Care Plan will be sent to any student with a medical condition which we ask you to return as soon as possible, to ensure their safety within school and to update us on any changes.
Important: In accordance with the Foundation First Aid Policy, parents must inform the School Nurse if their child requires prescribed medication at school, whether that is for short or long term treatment, so that we can discuss how this will be administered safely. All prescribed medication must be provided in its original packaging with prescription label attached.
WELFARE
If you have specified yes, our Designated Safeguarding Lead will arrange a call with you prior to your child's start with us.
Consent for Trips and Off-Site Activities

Taking pupils out of school places a serious responsibility on the members of staff in charge. It is therefore vital that they can trust students to behave in an appropriate and acceptable manner at all times. Parents should be aware that, should there be any serious misbehaviour prior to the trip, permission to go will be withdrawn and the costs incurred will be borne by parents/guardians. If there are any serious breaches of discipline or unacceptable behaviour, as determined by the person in charge, whilst away from school, then the student(s) concerned will be disciplined and possibly sent home at the parents' expense.
I also consent for personal data, including emergency contact details, medical and dietary information, care plans and other relevant information to be shared where appropriate with trip providers/ third parties. This information will only be shared with third parties when necessary to ensure the safety of our pupils and will be stored and destroyed by the third party in accordance with their privacy notice. We seek assurances from third party providers that personal data will be kept and destroyed securely and in a timely manner.
I understand that it is my responsibility to inform the school, as soon as possible, if any relevant personal or medical information changes. I appreciate it is essential that all student records are kept up to date for health and safety reasons.
Please confirm below that you are the parent/legal guardian of the above child and, understanding the above, hereby grant and confirm permission for them to undertake educational trips, visits, off-site activities, whole school and school related events from 1 September 2025 to 31 August 2026.
Use of Images of Pupils by the School

The
School makes various uses of images of pupils during their time at school as
set out in our Privacy
Notice, CCTV Policy and Use of Images
of Pupils Policy. While we will not always require your consent for
such uses, we do want to ensure that you are both properly informed about how
your child’s image will be used and fully understand your choices.
Essential use of images
Some
uses are necessary for education, administration and the safety of pupils, such
as (CCTV and for your child’s school security ID). Generally, the school relies
on its legitimate interests for such purposes.
Internal use of images within the school
community
Other
uses may not be strictly necessary, but do contribute to our school community,
and we anticipate that most parents and pupils will want to support such use.
These include images of pupils in school uniform or games kit, but never
swimming costumes, at work or playing games or other activities, for: internal
use within the school community: for example, internal school mail, school
magazines, notice boards, printed or digital newsletters and FireFly.
Again,
we consider these legitimate interest uses. We ask you to confirm below that
you understand we may use images of your child for these purposes and to let us
know if you object to such use.
External use of images
We
may also use images of pupils to celebrate achievements and promote the school
externally, in printed publications and online, including on the School website
or on its social media channels e.g.
Facebook, Instagram and X (formerly) Twitter. This generally involves photos of
pupils in school uniform or games kit, but never in swimming costumes, at work
or playing games or other activity. Care will be taken to limit the scope for
any child to be prominently featured in such images, unless this is consented
to and appropriate in the circumstances. Please note, we are not seeking
consent for images where your child cannot reasonably be identified to anyone
outside the school community.
We
ask below for your general consent to the use of images of your child in (i)
printed material (such as the school magazine or prospectus) and/or (ii) for
certain limited online or external media uses (where your child may be featured
as part of a group shot, but is not a prominent or central focus of an image).
From
time to time the school may seek a specific additional consent from you
(and/or, as appropriate, your child) for any more prominent online or external
media uses. These will as far as possible be specifically described to your
and/or your child before being published.
Objection, withholding or withdrawal of
consent
Please
be aware that objecting or opting out does not prevent the School from
continuing to process images of your child that are essential (as set out
above). If you do object to non-essential internal uses, these will be
considered by the school, and reasonable steps will be taken (for example, to
digitally blur or crop group shots). You can always opt out of marketing uses,
although it may not be possible to recall materials that already have gone to
physical publication based on earlier permissions
Internal Use of Images within the School Community
Tempest Photography
In order for us to make use of the contactless photography service, please could you authorise us to share your email address and mobile number, along with pupil ID, name and class. This will then authorise Tempest Photography to send your child's school photograph directly to you for you to place an order.
